At the Community Health Center Dental Clinic, our staff is dedicated to improving dental health for you and your family. Our dental clinic provides you with comprehensive health care while assisting you with an appropriate and individualized treatment plan.

Below, you will find the patient paperwork that needs completed before scheduling a dental appointment. Please print, fill out, and return paperwork using one of the avenues listed below.

Dental Records Release: If you have seen a dentist within the last 5 years, please complete the form below in addition to your registration paperwork. This form allows us to request your x-rays from a previous office and avoid an overcharge to you.

When returned, please include a copy of your dental insurance card (front and back). If you do not have dental insurance be sure to mention this to the patient support staff when scheduling your appointment to discuss payment options and the sliding fee scale.
