By combining the healing touch of skilled caregivers with sophisticated, secure information technology, we are redefining how patient care is delivered and how you access important information about your care.

Connecting with your healthcare information is easy when you create an account to access our secure on-line patient portal. Available to you anywhere you have an Internet connection – or by using the convenient Healow app on your smartphone or tablet – our patient portal will give you access to:

  • Laboratory test results
  • Medication summaries
  • Visit and procedure summaries
  • Immunization summaries
  • Educational materials and resources

The patient portal also allows you to:

  • Send secure messages to your healthcare team
  • Manage health information for your children and family

How to Register for Patient Portal Access

During your next visit to the CHCCW Health Center where you receive care, tell the staff that you want to register for a patient portal account. You will be asked to provide a personal email address and a secure message will be sent to the address you provide. The message will guide you through the necessary steps to complete the registration process.

You will need to develop a unique username and password for your account. Please make sure you keep this information in a secure location.

Once you have completed the registration process, you can access your health information via the Internet or by downloading the Healow app from the App Store or Google Play.

Using the Healow App

  • Download the Healow App from the App Store or Google Play.
  • When you launch the App, you will be asked to agree to the terms of service.
  • Enter your first and last name and date of birth when prompted.
  • You will be prompted to enter a practice code. Use ACJGAD for Community Health Center of Central Wyoming.
  • You will then be asked to enter the unique username and password for your portal account.
  • Now follow the instructions to designate a PIN to access your health information and you are ready to go.

Access to the portal is provided for your convenience, free-of-charge, 24 hours a day, seven days a week. If you need access to information from your medical record but are uncomfortable using the Internet, please contact the CHCCW location where you receive care. Appointments requested through the patient portal are subject to availability. We recommend that you contact the CHCCW Health Center where you receive care to schedule an appointment.

The portal should NOT be used for urgent or emergency communications. Communication through the portal is not intended to replace an office visit. Please do not send messages through the portal that require immediate medical attention. If you do not receive a response within 72 hours, you should contact the office.

The portal is compliant with the Health Insurance Portability and Accountability Act HIPAA), maintains the highest level of confidentiality, and provides you with the ability to securely access information in your medical record.
