Chronic Care Management helps keep you healthy by providing ongoing support and coordinated care, so you can focus on doing what you love without the stress of managing chronic conditions alone.

Chronic Care Management services are available to our patients who are insured through Medicare and are living with two or more chronic health conditions. These services are designed to provide personalized, continuous support to help manage your health more effectively, reduce complications, and improve your overall quality of life. By working closely with your care team, you can stay on track with your treatment plan and maintain better control over your conditions.

What is a Chronic Condition?
A disease or illness that is persistent, lasting 12 months or more, and may lead to a gradual deterioration of health and/or loss of independence.
Examples of chronic conditions may include:

  • Diabetes
  • Hypertension
  • COPD
  • Asthma
  • Heart Disease
  • Arthritis
  • Alzheimers/Dementia
  • Depression
  • Cancer

How Chronic Care Management will Benefit You
CCM is a patient-centered care program designed to help guide you in your health and wellness journey. It is a monthly service which may include:

  • Assisting you with creating an individualized care plan.
  • Ensuring recommended preventive services are utilized.
  • Coordinating care with your providers, pharmacy, specialists, and more.
  • Identifying and arranging for community resources.
  • Coordinating with home and community-based providers.
  • Assisting in care transitions.
  • Home visits.
  • Telephone calls.
  • Skilled and caring support to help with setting, meeting, and maintaining your health goals.
