Prepare your student athlete for their chosen sport with a no cost to you sports physical at Community Health Center of Central Wyoming (CHCCW).
Sports physicals, also known as pre-participation physical examinations, are required to determine if young athletes are healthy and well enough to participate in athletic activities. We can help by providing the required physical examination at no cost to you.

Dubois

If you have health insurance, please bring a copy of your insurance card. Student athletes under the age of 18 can come without a parent or guardian, only if the paperwork is filled out by the parents prior to the appt. (Only for the Dubois Clinic)
We will bill Medicaid or Private Insurance for all Vaccines & Administration Fees. There will be an administration Fee of $35.00 if scheduling a Sports Physical NOT on May 14 or July 30.
Lander
