New at CHCCW
- New Clinical Model: This new model established
four separate, but identical, "pods" of clinical care
teams. This new system has increased staff efficiency and maximized
patient services, resulting in better patient outcomes. Patients
now check-in/out, handle financial transactions, schedule
appointments,
and interact with the same staff members throughout their visit
- creating a more patient-centered atmosphere.
- The Health Disparities Collaborative: will spread
to include the Cardiovascular Collaborative during Spring 2007.
CHCCW was nationally recognized for their outstanding achievements
in the Diabetes and Depression Collaboratives for 2005.
- Free Mammograms - women over 40 or with risk factors
for breast cancer who meet income guidelines are eligible. This
program is provided with funding from the Wyoming Affiliate of
the Susan G. Komen Breast Cancer Foundation. Contact the Women's
Health Coordinator at 233-6015 for more information.
- New Payment Procedures-patients must pay for services
or make arrangements for payment before seeing the physician or
other health provider. This procedure reduces the time patients
must spend in the clinic and better utilizes the time spent waiting
to be seen by a provider.
